Evening Shift LPN (Licensed Practical Nurse)
Job Summary:
Brookmont Health & Rehab, a trusted senior care community in Effort, Pennsylvania, is seeking a dedicated and compassionate Evening Shift LPN (Licensed Practical Nurse) to join our team. As a valued member of our healthcare staff, you will play a vital role in providing high-quality nursing and rehabilitation services to our residents. Responsibilities:
• Provide direct patient care to residents on the evening shift, including administering medications, wound care, and other nursing interventions
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report resident conditions, progress, and concerns to the nursing staff and other healthcare professionals
• Collaborate with other healthcare professionals to ensure continuity of care and effective communication
• Participate in quality improvement initiatives to enhance the overall quality of care and services
• Maintain accurate and thorough records of patient care and experiences

Requirements:
Valid LPN (Licensed Practical Nurse) license in the state of Pennsylvania
Current CPR certification
Experience in a long-term care or rehabilitation setting (not required but desirable)
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment with attention to detail and prioritization skills
