Case Manager/Independent Living Coach – Cocoa This role combines case management responsibilities with independent living coaching to support participants as they meet program and operational objectives. The candidate will be responsible for determining services and housing eligibility, creating and adjusting Treatment/Life Plans, and ensuring participants follow these plans toward measurable and attainable outcomes. Responsibilities Assist participants to meet program and operational objectives. Determine services and housing eligibility needs based on Treatment/Life Plan implementation. Develop Treatment/Life Plans that are easily followed, measurable, and attainable. Work independently with initiative and without close daily supervision. Qualifications Bachelor’s Degree in Human Services or a related field. At least one year of experience working with an adult population. Strong organizational skills and ability to develop clear, concise, and actionable Treatment/Life Plans. Ability to work effectively and autonomously without close daily supervision.
