Case Manager I | Make a Real Impact in Your Community Location: Sacramento, CA Schedule: Full-Time Pay: $24.90 per hour Are you passionate about helping others overcome barriers and improve their quality of life? We’ re seeking a dedicated Case Manager I to support individuals facing challenges such as housing instability, mental health conditions, substance use, and chronic illness. This is a highly impactful, community-based role where you’ ll build meaningful relationships and connect clients to essential resources that change lives. What You’ ll Do Manage a caseload of ~50 clients and provide individualized support Conduct assessments and develop goal-oriented care plans Connect clients to housing, healthcare, income, and community resources Provide in-person support (70% field-based, 30% office-based) Advocate for clients and coordinate care with partner organizations Document services and maintain accurate records in EHR systems What We’ re Looking For High School Diploma or GED (Associate’ s preferred) Experience working with underserved populations (lived experience welcomed) Strong communication, organization, and time management skills Ability to thrive in a fast-paced, field-based environment Valid driver’ s license, reliable vehicle, and willingness to transport clients CPR/First Aid certification (or willingness to obtain within 90 days) Why You’ ll Love This Role Competitive pay + excellent health benefits at reduced cost 11 paid holidays + generous PTO & sick time Retirement plan with company match (no vesting!)
