Location: Sacramento, CA 
Schedule: Full-Time
Pay: $24.90 per hour 


Are you passionate about helping others overcome barriers and improve their quality of life? We’ re seeking a dedicated Case Manager I to support individuals facing challenges such as housing instability, mental health conditions, substance use, and chronic illness.

This is a highly impactful, community-based role where you’ ll build meaningful relationships and connect clients to essential resources that change lives.

What You’ ll Do

• Manage a caseload of ~50 clients and provide individualized support
• Conduct assessments and develop goal-oriented care plans
• Connect clients to housing, healthcare, income, and community resources
• Provide in-person support (70% field-based, 30% office-based)
• Advocate for clients and coordinate care with partner organizations
• Document services and maintain accurate records in EHR systems

What We’ re Looking For

• High School Diploma or GED (Associate’ s preferred)
• Experience working with underserved populations (lived experience welcomed)
• Strong communication, organization, and time management skills
• Ability to thrive in a fast-paced, field-based environment
• Valid driver’ s license, reliable vehicle, and willingness to transport clients
• CPR/First Aid certification (or willingness to obtain within 90 days)
