Responsibilities:

• Provide direct patient care and support in accordance with your licensure and certification level
• Assist patients with activities of daily living (ADLs) including bathing, grooming, and mobility
• Monitor and document patient health status, vital signs, and changes in condition
• Administer medications and treatments as directed (RN/LPN)
• Collaborate with care teams to develop and implement individualized patient care plans
• Offer emotional support and companionship to patients and their families
• Maintain a safe, clean, and comfortable environment for patients

Requirements:

• Active and valid RN, LPN, HHA, PCA, or CNA license or certification in New Jersey
• Prior experience in home health, assisted living, or clinical care preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a collaborative care team
• Reliable, punctual, and committed to delivering high-quality patient care
• Compassionate, patient-centered approach to caregiving
• Ability to pass a background check and meet all state compliance requirements
