Team members are expected to uphold these values in every interaction and service provided. Role Description This full-time, on-site Home Health Aide role is based in Carol Stream, IL. The Home Health Aide will assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility support. Responsibilities include providing personal care, monitoring clients’ well-being, supporting safe movement around the home, and following individual care plans. The role also involves light household tasks related to client care, such as meal preparation, maintaining a clean environment, and documenting care activities. The Home Health Aide will communicate regularly with clients, families, and supervisors to report changes in condition and ensure coordinated, compassionate care that aligns with the company’s mission. Qualifications Demonstrated skills in home care and patient care, including following care plans and safety procedures. Ability to support activities of daily living and personal care, such as bathing, dressing, grooming, and toileting. Strong communication skills to interact effectively with clients, families, and care team members. Compassionate, reliable, and patient-focused approach with respect for diverse backgrounds and abilities. High school diploma or equivalent; completion of a Home Health Aide or Certified Nursing Assistant (CNA) program preferred. Previous experience in home care or a similar direct care role is beneficial. Ability to lift, move, or assist clients safely, and to meet the physical demands of in-home care.
