Case Manager & Support Coordinator for Independent Living
Care Lync
Care Lync, a Social Services agency, is seeking a dedicated Support Coordinator to manage individualized care plans and coordinate services for adults with IDD, Autism, and physical disabilities enrolled in Medicaid Waiver programs. You will develop ISP-based plans, broker services, and monitor outcomes to support independent living.
This role requires at least a BS/BA and 3 years of case management experience, proficiency in Microsoft Office, and valid driver’s license.
