Mental Health Case Manager & Family Services Facilitator
ALL OF GODS CHILDREN GROUP STRTP
All of God’s Children will implement the Core Practice Model to improve engagement, assessment, treatment planning, and intervention services for children, families, and communities. The CFTM Facilitator leads the Individualized Child and Family Plans and coordinates service delivery within a supportive team. Responsibilities include coordinating client intake, scheduling visits, transporting clients to appointments, and ensuring all required forms and documentation are completed and updated #J-18808-Ljbffr ALL OF GODS CHILDREN GROUP STRTP
