Staff Pharmacist (RPh)

Northern Pharmacy and Medical Equipment is seeking a licensed Staff Pharmacist to join our pharmacy team.

We are looking for a knowledgeable, dependable, and patient-focused pharmacist who is committed to medication safety, clinical excellence, and exceptional service. The Staff Pharmacist will be responsible for reviewing and verifying medication orders, providing clinical recommendations, ensuring regulatory compliance, and collaborating with physicians, nurses, pharmacy technicians, and other members of the healthcare team.

Experience in Long-Term Care pharmacy is preferred.

Requirements:

 

Responsibilities

• Evaluate physician medication orders for appropriateness of drug selection, dosage, potential drug interactions, route of administration, and stability. 
• Provide clinical recommendations regarding medication therapy changes to physicians and facility staff when appropriate. 
• Review patient medical information and conduct Drug Utilization Reviews (DUR) on prescriptions to identify potential drug interactions, allergies, contraindications, and other clinical concerns. 
• Review and resolve clinical edits as appropriate. 
• Verify the completeness and accuracy of new and refill prescription labels and medication orders. 
• Verify the content and accuracy of medication packaging and labeling, including emergency drug kits. 
• Maintain resident information within the pharmacy database, including relevant current and past medical history, clinical and laboratory data, and concurrent medications, including OTC medications and home remedies. 
• Assist with the proper ordering, handling, receiving, documentation, and storage of controlled substances. 
• Assist in maintaining perpetual inventory records for Schedule II controlled substances. 
• Work collaboratively with pharmacy technicians to reconcile controlled-substance perpetual inventory as required by management and in accordance with state and federal regulations. 
• Provide clinical expertise by responding to medication-related questions and concerns from internal staff and external customers. 
• Communicate with physicians, nurses, patients/residents, family members, and other members of the healthcare team as appropriate. 
• Document relevant clinical communications and interventions in the appropriate patient/resident record. 
• Maintain compliance with applicable federal and state pharmacy regulations, company policies, and HIPAA requirements. 
• Participate in evening, weekend, and holiday coverage rotations as required. 
• Participate in the pharmacist on-call rotation approximately every third week and as needed . 
• Perform other duties as assigned based on departmental and business needs.

 

Minimum Qualifications

• Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) degree from an accredited School of Pharmacy. 
• Current Maryland Pharmacist license that is active and in good standing. 
• Current immunization certification. 
• Current CPR certification. 
• Ability to accurately and efficiently review, verify, and dispense prescription medications. 
• Strong clinical judgment and attention to detail. 
• Effective verbal and written communication skills. 
• Ability to communicate professionally with physicians, nurses, patients, caregivers, facility staff, and pharmacy team members. 
• Ability to read, write, speak, and understand English. 
• Ability to work flexible hours, including evenings, weekends, and holidays. 
• Ability to participate in the required on-call rotation. 

Preferred Qualifications

• Long-Term Care pharmacy experience. 
• Experience with QS/1 pharmacy software. 
• Experience with DocuTrack . 
• Proficiency with Microsoft Word, Excel, and Outlook.
