THIS WORK MATTERS! At Corrections Health, we provide medically needed health care and transitional services to adults and youth in custody at Multnomah County Jails and Juvenile Detention Facility. The Advanced Practice Clinician (APC) will provide Medication Assisted Treatment (MAT) cooperatively with the Corrections Health and Harm Reduction Program teams. The APC will provide effective and efficient evaluations, diagnosis, treatment plans and prevention services for those with substance use disorder or who are referred for MAT while in custody. The APC must consistently demonstrate effective, positive and respectful communication with patients and staff including active listening, empathy, courtesy, cultural sensitivity, and the ability to promote modification of health behaviors with clients. This position will work in both Corrections Health and Harm Reduction Program locations, with service hours and schedules that meet the needs of the population served. As an Advanced Practice Clinician in Corrections Health and the Harm Reduction Program team, you will serve at two of our Correctional Health facilities and the Menlo Park Syringe Service Program and Clinic (HRC): Multnomah County Detention Center (downtown) Inverness Jail (near PDX airport) Harm Reduction Clinic at Menlo Park Plaza (NE Portland) Responsibilities Clinical Care Formulate appropriate diagnoses and implement treatment plans based on HRC practice guidelines, Corrections Health (CH) policies, Health Department policies, and state and MCHD stage-based behavioral counseling guidelines. Provide ultra low barrier buprenorphine and other medication access to support recovery and reduce risk of overdose. Provide health education to patients. Perform wound care and care of skin and soft tissue infections, including chronic conditions such as venous stasis ulcers. Provide vaccinations as outlined in HRC clinical practice guidelines. Offer contraception services and HIV PrEP based on HRC Program clinical guidelines. Maintain exam room and orders and review appropriate diagnostic tests and medications. Perform sexual health history, physical examination and specimen collection as outlined in HRC Program and Corrections Health clinical guidelines. Perform sexual health screening, genital and gynecological exams, and diagnosis and treatment of genital and sexually transmitted infections (STI). Perform venipuncture as needed. Order and interpret laboratory studies. Prescribe medications such as contraception, HIV Pre-Exposure Prophylaxis (PrEP), doxy-PEP, and antibiotics. Provide follow-up patient consultations, telehealth visits, and other medical conditions. Develop and direct patient-centered care plans with the assistance of other disciplines within the care team. Refer clients to the physician when beyond scope of practice. Consultation and Care Coordination Provide consultation to nursing staff, Menlo Park Harm Reduction Clinic program staff, Health Department Clinics, and community providers as indicated for continuity of client care. Assist patients regarding additional care needs, including preventative healthcare, specialty or rehabilitative services, or high risk / acute care. Provide in-service training to staff as requested. Partner with the Senior Harm Reduction Specialist and Corrections Health to enhance and improve MAT services across relevant sites. Participate in precepting students, volunteers, and new staff when appropriate. Make appropriate referrals to care in coordination with the patient and care team. Assist in developing and implementing protocols, procedures and other teaching tools for employees. Provide direction to Community Health Workers, Community Health Nurse and Clinical Service Specialists to implement aspects of care plans within their scope and training. Documentation and Reporting Review client records for needs for medications, treatments, education or referrals. Document appropriately and clearly in the electronic health record (EHR) system. Utilize panel based information technology tools to prioritize and direct proactive care for patients assigned to the provider panel. Apply division, department, and all healthcare requirements for HIPAA and other compliance standards. Partner with department and evaluation staff to identify priority MAT outcomes and workflows for evaluation and improvement. Minimum Qualifications/Transferable Skills Master's Degree in Nursing AND one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position. License required to practice medicine as a Nurse Practitioner or Physician Assistant as outlined by Oregon Revised Statutes (ORS) 678.375 or (ORS) 677.515. CPR certification required. DEA License. Previous experience prescribing buprenorphine for the treatment of substance use disorder is required. Passing a Multnomah County Sheriff's Office background check is required. Preferred Qualifications/Transferable Skills Experience working in a correctional facility. Training or experience in working with Electronic Health Record systems. Experience working with targeted groups and with ethnically and culturally diverse populations. Experience in medication assisted recovery (e.g. medications for opioid use disorder, MOUD, MAT, or stimulant use disorder treatment). Experience in opioid withdrawal management. Demonstrated understanding, implementation, and promotion of harm reduction and social justice principles.
