Role: Part-Time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to improve patient health outcomes. Responsibilities: Conduct 45-minute in-home or virtual visits, reviewing medication and medical history, performing physical evaluations, and ordering diagnostic tests if needed. Build trusted relationships with patients, deliver personalized care, and utilize digital workflows without prescribing medication or altering treatments. Qualifications & Skills: Active, unrestricted licenses, board certification, multi-state licenses preferred, and ability to cover specific locations. Strong communication and compassionate engagement are essential.
