Job Summary The Social Worker position provides comprehensive, compassionate, clinical social work and discharge planning to patients and families of a targeted patient population on a designated unit(s). The role involves educating patients on treatment, health, and wellness choices while facilitating psychosocial services. Responsibilities Provide comprehensive, compassionate, clinical social work to patients and families. Conduct discharge planning for patients on the unit. Educate patients about treatment options, health, and wellness choices. Deliver psychosocial services to dialysis patients, both in-center and at home. Utilize Social Work Theory of Human Behavior and accepted methods to support patients. #J-18808-Ljbffr White Lodging
