• Experience: Minimum 1 year outpatient geriatric and primary care experience

What You ll Do

• Provide outpatient primary care to geriatric patients, managing chronic and complex health conditions
• Conduct comprehensive geriatric assessments and evaluations
• Develop and implement care plans focused on preventive and chronic disease management
• Collaborate with healthcare teams to coordinate patient care
• Document clinical encounters accurately in Epic EMR
• Offer health promotion and education tailored to senior patients
• Participate in after-hours call coverage (approximately one week per year, including evenings and weekends)

What You NeedRequired

• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Minimum 1 year outpatient clinical experience
• Successfully pass credentialing ( 4 months process)
• Able to manage geriatric health concerns confidently
• Strong documentation skills

Preferred

• Experience with outpatient geriatric management
• Comfort working in a clinic setting with an established senior patient population
