Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers for individualized care plans
• Identify and address cost-efficient care levels
• Ensure compliance with treatment plans by arranging non-medical support
• Coordinate care transitions as needed
• Engage specialty resources to achieve optimal resolution
• Keep detailed records of outcomes during management

Requirements of the RN - Case Management

• KY RN License or Compact RN License
• HCHB experience
• 1 year home health experience
• Associates or Bachelor's Degree in Nursing or related field
• Case Management certification
