Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers to implement individualized care plans.
• Address and reassess cost-efficient levels of care continuously.
• Facilitate non-medical support to ensure compliance with treatment plans.
• Coordinate care transitions as necessary with appropriate professionals.

Requirements of the RN - Case Management

• KY RN License or Compact RN License
• 1 year home health experience
• HCHB experience
• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Ability to analyze complex medical information and make rational decisions
• Case Management certification
