Job Requirements

• Credentials:
• • State Licensure
  • Current BLS

• Health Documentation:
• • Immunizations: Varicella, Rubeola, Rubella, Mumps
  • Physical within the last twelve months
  • Negative TB within the last year or
  • Clear Background Check (provided)
  • Negative Drug Screen (provided)
