Provide direct patient care to residents, including administering medications, wound care, and vital signs monitoring

Develop and implement individualized care plans, working closely with the healthcare team to ensure comprehensive care

Conduct assessments, document patient progress, and communicate effectively with colleagues and family members

Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Collaborate with other healthcare professionals to ensure efficient and effective care delivery

Current licensure as a Licensed Practical Nurse (LPN) in the state of Illinois

High school diploma or equivalent required; associate's degree in nursing or related field preferred

Ability to lift, transfer, and reposition patients; must be physically able to perform duties without assistance

$33 - $38 HOURLY
