Home Health Physical Therapist

Join a caring community as a Home Health Physical Therapist Help patients regain strength, mobility, and independence through personalized, home-based care focused on rehabilitation and functional improvement. You will collaborate with physicians and the clinical team to develop, implement, and revise therapy plans, administer skilled physical therapy, and oversee progress reports and discharge summaries. Provide clear instructions to Therapy Assistants and Home Health Aides, monitor their performance, and report patient progress to relevant stakeholders. Qualifications Include Current, unrestricted Physical Therapy license in North Carolina Valid driver’s license, auto insurance, reliable transportation Current CPR certification Preferred experience with Home Health is advantageous. Key Duties Develop and adjust treatment plans Administer therapy to improve physical function Coordinate with healthcare professionals and document patient progress This role offers a competitive salary, benefits supporting well-being, and opportunities to make a meaningful impact in patients’ lives.
