Sage Health is seeking a Market Medical Director to lead physician performance, clinical quality, and value-based care across our markets. This is a hands-on physician leadership role for someone who wants to coach exceptional PCPs, improve outcomes, and remain connected to patient care. The Opportunity As Market Medical Director, you'll lead physicians across your market while maintaining a regular clinical presence. You'll serve as a physician coach, clinical leader, practicing clinician, and partner to operations. You'll: Lead physician performance, engagement, retention, recruitment, and onboarding. Coach and develop PCPs in clinical excellence and value-based care. Drive market performance across quality, HEDIS, STARS, CAHPS, risk adjustment, access, utilization, and patient engagement. Partner with operations to improve workflows, access, staffing, and team performance. Support PCPs caring for medically and socially complex patients. Provide approximately one half-day per week of direct patient care, based on market needs. Build clinical best practices and develop future physician leaders. Build relationships with specialists, health systems, and community partners. Qualifications MD or DO with active or eligible state licensure. Board certified/eligible in Internal Medicine, Family Medicine, Geriatrics, or Med-Peds. 5+ years of clinical experience caring for medically complex adults. Physician leadership experience. Experience with Medicare Advantage, ACO, or value-based care. Strong knowledge of CMS quality programs, risk adjustment, and population health. Excellent communication, coaching, and team-building skills. Preferred: Multi-site physician leadership, fully capitated Medicare Advantage experience, and familiarity with HEDIS, STARS, CAHPS, RAF, and utilization management.
