We are seeking a Licensed Social Services Worker to join our team! Identify social and emotional needs for residents. Provide medically related social services to attain or maintain the highest practical physical, mental, and psycho-social well-being of each resident. 

Responsibilities:

• Ensures the accuracy and completion of resident admission tasks including correspondence and facility tours.

• Oversees the transfer of residents to/from home or to another facility.

• Serves as the Ombudsman for the residents.

• Attends resident council meetings, care plan meetings, and other appropriate committee meetings.

• Ensures social service assessments are completed for each resident resulting in Individualized Care Plan.

• Monitors and controls social service progress notes

• Coordinates the assignment of outpatient appointments, including transportation.

• Ensures residents' personal needs are met

• Manages social services assistants to ensure accuracy and completion of assigned tasks

Qualifications:

• Master’s degree in social work.

• Two years of experience working in geriatrics.

• Valid Driver’s license.

• Must have state Licensure

• Previous experience with Nursing Homes preferred.
