Nurse Practitioner (NP) in Physical Medicine & Rehabilitation (PM&R)

We are seeking a dedicated and compassionate Nurse Practitioner (NP) specializing in Physical Medicine & Rehabilitation (PM&R) to join our growing team in Charlotte, NC. In this full-time role, you will provide high-quality, patient-centered care to geriatric and rehabilitation patients within a skilled nursing facility (SNF). This is an excellent opportunity for an NP who values autonomy, collaboration, and making a meaningful impact on patient outcomes and quality of life.

Key Responsibilities:

• Deliver comprehensive PM&R care to geriatric and post-acute patients.
• Collaborate with interdisciplinary teams including nursing, therapy, and facility administration.
• Develop and implement individualized treatment and preventative care plans.
• Build strong relationships with patients, families, and facility staff.
• Lead initiatives to reduce hospitalizations and improve functional outcomes.
• Ensure continuity of care and clinical excellence across the facility.

Compensation & Benefits

• Salary: $115,000 – $140,000 annually (highly competitive).

Required Qualifications:

• Active North Carolina Nurse Practitioner License
• DEA License
• CDS License
