Job Title: Case Manager Nurse Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum 2 years of Acute Care Case Management/Discharge Planning License: Valid CA state license required, Pending license accepted (must be licensed upon start date) Certifications: Not specified Must-Have: Must have hospital experience. Behavioral health experience does not apply. Every other weekend availability is required. Description: The Case Manager Nurse is responsible for overseeing and coordinating patient care. This includes care coordination, discharge planning, and utilization review. Experience in assessing patients and developing treatment plans is essential. The ability to organize and participate in family meetings is required. Knowledge of advance directives, POLST, and protective services processes is important. Competency in arranging Durable Medical Equipment (DME), skilled nursing facilities (SNF), and ambulance transport is necessary. The nurse must work independently and perform tasks effectively. Traveling may be required between nearby facilities. Candidates must commit to working three out of five major holidays. It is crucial to respond to scheduling requests upfront. Non-compliance with scheduling or refusal to float may lead to termination of the contract. Candidates should possess skills in assessing patients, care coordination, and case management processes. Knowledge of various insurance models and patient assistance options is highly valued. An understanding of diverse patient backgrounds and unique treatment options is required. Interdisciplinary support within the unit includes multiple health professionals. The daily census and patient care dynamics will reflect a variety of patient age groups. Experience with computerized documentation is required for this role.
