The Occupational Therapist-Home Health in northern Scottsdale plays a vital role in delivering personalized care and rehabilitation services to primarily adult patients within their homes. Working as part of an interdisciplinary team, this therapist conducts assessments, develops tailored care plans, implements therapy, and provides patient education to enhance patients' functional independence. Frequent travel to multiple homes is required to ensure comprehensive therapy delivery and ongoing progress monitoring.

Responsibilities

• Conduct thorough patient assessments to evaluate functional abilities and needs
• Develop and implement individualized care plans targeting patient goals
• Deliver therapeutic interventions in home settings to improve daily living skills
• Perform regular home visits, adapting therapy based on patient progress and environment
• Educate patients and caregivers on therapeutic techniques and safety practices
• Monitor and document patient progress using electronic health records
• Collaborate closely with interdisciplinary team members to coordinate patient care
• Evaluate home environments for safety and recommend modifications as needed

Preferred Qualifications

• Minimum 1 year of experience in occupational therapy practice
• Licensed Occupational Therapist (OT)
• Proficient in patient assessment, care planning, and therapeutic techniques
• Experience in home health care delivery
• Strong communication, time management, and problem-solving skills
• Demonstrated empathy and patient-centered approach
