Nurse Case Manager (Registered Nurse)

Nurse Case Manager (Registered Nurse) Program: Enhanced Family Foster Care | Reports to: Nursing Director Location: Brooklyn, NY, US Employment: Full Time, Hybrid – 3 days onsite (10:00 AM‑6:00 PM) Salary Range: $45.71 to $46.70 per hour Reporting to the Nursing Director, the Nurse Case Manager will be accountable for the full patient‑care cycle while the child is in foster care. The successful candidate will coordinate children and youth’s medical and mental health care and provide resources to foster independent living. The nurse encourages good health practices and preventative health care with foster parents and children/youth. Key responsibilities include: Responsibilities Assess, plan, implement, and monitor the overall health care of foster children, coordinating timely care by developmental milestones Establish effective working relationships with birth parents, foster parents, case‑work staff, schools, community providers, and all team members to ensure safety and optimal health Schedule required medical and specialty appointments within requested and mandated timeframes; conduct assessments, including developmental, HIV risk, intake, general first aid, and triage activities Conduct routine screening for substance abuse, developmental health, mental health, and physical body checks for abuse Identify physical signs of physical abuse and indicators of sexual abuse Participate in on‑site well‑child clinics, documenting vital signs, education and assisting the agency pediatrician Provide routine management, training, teaching, and health education, including reproductive health education for children and youth Manage chronic conditions such as diabetes and asthma; provide education and training for children, youth, and foster parents Support and manage the emotional/psychiatric needs of children Establish comprehensive treatment goals using current information and collaborating with the multidisciplinary team Follow up after medical appointments, obtain reports, and ensure urgent/emergency care or hospitalization follow‑up within 24 hours Maintain accurate medical documentation, including timely progress notes Assist with home visits and community provider visits as needed Communicate promptly with the child/youth’s treatment team, including medical, case planning, mental health, external providers and other specialties Ensure all emails and phone calls are answered within 1 business day Follow regulatory requirements from ACS and OCFS Assist with maintaining medical supplies and equipment Practice proper infection control Proficient with technology, including Microsoft software and internal EHR/Connections Other duties as assigned Special Qualifications Experience in the child welfare system Bilingual capability – English/Spanish (plus) Knowledge of foster care health care requirements Excellent problem‑solving, multitasking, and communication skills Attention to detail, compassionate, team‑player, adaptable, proactive, and motivated Experience working with MCOs Familiarity with Netsmart/ MyEvolve (plus) Minimum Requirements Valid and unrestricted nursing license Bachelor of Science in Nursing (BSN), RN or LPN Certification by the American Red Cross (or equivalent) in BLS with First Aid, or ability to obtain before hire Minimum two years of work experience (shorter periods considered if related) The statements herein are intended to describe the general nature and level of work being performed by the incumbent in this position.
