About this role:

As anAdvanced Practice ProviderHome InstaCare,you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In thisrole,you willprovide Urgent/ InstaCare services to patients in their home.

• This position includes a $5,000 sign-on bonus , up to $7,500 in relocation assistancewhen applicable, a competitive salary, andan $18,000.00 Annual Quality Incentive

• Your home base will be Clinic Wynn this role isprimarilyamobile practicedelivering care to the patient in their homesettingvia an Intermountain car/vehicle

• Weekly schedule involves 3 x 12 hour days/ week

Whatyoullbring:

• 2+ Years Experience as a practicing Advanced Practice Provider

• APRN or PA-C degreefrom an accredited program

• Board certification or eligibility inthe designated discipline

• Active, unobstructedNevadamedical license, or the ability to obtain one

• Full prescriptive authority and an active DEA, or the ability to obtain one

• Ability to successfully complete Intermountain Healths credentialing process
