LTSS Service Coordinator (Case Manager)

$26.19 - $32.75 per hour

Job Title LTSS Service Coordinator (Case Manager) Location & Eligibility Statewide across Ohio. Must reside in the state of Ohio. The primary setting is field-based, traveling to client sites or designated locations. Occasional office attendance for meetings or training. Alternate locations may be considered for candidates residing within commuting distance of an office. Key Responsibilities Lead the Person Centered Planning process, documenting member preferences, needs, and goals. Conduct face-to-face program assessments using predefined tools and motivational interviewing techniques. Develop and implement comprehensive Person Centered Support Plans (PCSP) and backup plans. Coordinate and manage member waivers such as LTSS/IDD, BH or PH needs, serving as single point of contact. Engage interdisciplinary care rounds, collaborate with medical directors, and establish fully integrated care plans. Oversee management of physical health, behavioral health, and long‑term services, ensuring compliance with state and federal regulations. Identify and manage non‑clinical needs for members with chronic illnesses, comorbidities, or disabilities. Submit utilization/authorization requests with supporting documentation aligned to individual care plans. Report critical incidents to internal and external parties such as state and county agencies (e.g., Adult Protective Services, Law Enforcement). Assist in appeals, fair hearings, grievances, and state audits, as well as participate in process improvement initiatives. Mentor, serve as subject matter expert, or preceptor for new staff and formal training of associates. Qualifications BA/BS degree (preferred in health, nursing, or related field) and at least 2 years of experience working with a social work agency, or equivalent education and experience. Strong preference for case management experience with older adults or individuals with disabilities. Knowledge of state and federal regulations governing long‑term services and supports. Salary & Benefits Hourly rate: $26.19 to $32.75 per hour.
