RN Hospice-Night Shift On-Call-{170118} Location: Colorado Springs, CO 80920. Travel in Colorado Springs/Woodland Park area. · Mileage: $0.45/mile reimbursement Pay: $56.14/hr. Schedule: Mon-Fri, Night Shift On-Call: 5:00pm-8:00am (shift ends Friday AM) Hours: Guaranteed 40 hours/week Overview We’re seeking an experienced Hospice RN Case Manager for a night shift on-call role supporting patients in the Colorado Springs/Woodland Park area. This is an on-site position with field travel and guaranteed full-time hours. Key Responsibilities Perform PRN/focused assessments for high-risk members Collaborate with providers to implement individualized care plans Identify and reassess appropriate, cost-effective levels of care Coordinate care transitions with physicians, social workers, discharge planners, and other stakeholders Engage specialty resources as needed to resolve patient needs Maintain detailed documentation of clinical/functional outcomes Required Skills & Qualifications Active, unrestricted Colorado RN license (or multi-state license) 1 year of hospice experience (required) Homecare Homebase (HCHB) & OASIS experience (required) Reliable transportation and current car insurance Ability to work on-site and travel locally as needed Preferred Case Management certification Keywords Hospice RN, RN Case Manager, Night Shift, On-Call RN, Homecare Homebase, HCHB, Care Coordination, Utilization Management, Colorado Springs RN, Woodland Parkaa415a4b-8b21-40fc-a65c-70d2b25ca29a
