Job Summary
Physical Therapist Outpatient (PT OP) – Critical Access Facility – Location unspecified. This position involves providing outpatient physical therapy services within a critical access healthcare setting. The role offers flexibility in scheduling shifts, with no weekend work required, and requires a minimum of one year of experience in outpatient therapy.
Job Details
- Certifications, Education, and Experience Requirements: BLS certification required; minimum 1 year of outpatient therapy experience; active Illinois license preferred.
- Schedule: 10-hour shifts, Monday through Friday, with a minimum of 40 hours per week, starting on 11/16/2026 and ending on 02/15/2027.
- Responsibilities: Deliver outpatient therapy services, including assessments, treatment planning, and interventions for adolescent, adult, and geriatric patient populations. Adhere to patient care standards and collaborate with interdisciplinary teams.
- Preferred Qualifications: Experience with charting systems such as Cerner, community hospital experience, and prior outpatient or critical access setting familiarity.
- Additional Information: Orientation will be virtual and at-home on Day 1; completion of required modules is mandatory by 4:00 PM CST of Day 1. Floating may be required within the facility or to nearby affiliates. Candidate must be pre-closed at submission, with SSN and DOB provided, and available for interview within requested windows.
