What You'll Do

• Provide psychosocial assessments for hospice patients and their families.
• Develop individualized care plans that address emotional, social, financial, and community resource needs.
• Offer counseling, crisis intervention, grief support, and emotional guidance to patients and loved ones.
• Assist families with advance care planning and end-of-life decision-making.
• Connect patients and caregivers with community resources and financial assistance programs.
• Collaborate closely with physicians, nurses, chaplains, aides, and the interdisciplinary hospice team.
• Maintain accurate, timely documentation in accordance with agency, state, and federal regulations.
• Participate in care conferences and contribute to high-quality patient-centered care.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program.
• Previous hospice, home health, palliative care, or healthcare experience preferred.
• Strong communication, counseling, and interpersonal skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Valid Arizona driver's license, reliable transportation, and current auto insurance.
