Occupational Therapist

Seeking licensed Occupational Therapists to provide personalized, one-on-one care to older adults in private homes and senior living communities. Responsibilities include conducting evaluations, developing care plans, delivering evidence-based treatments to enhance mobility, independence, and overall well-being, and documenting care within EMR systems. Collaborate with interdisciplinary teams of PTs, SLPs, and fitness specialists, while educating patients, caregivers, and staff on safety and aging-in-place strategies. Flexibility to incorporate telehealth and virtual care expands service reach and continuity. The role offers autonomy to create treatment schedules, build strong patient relationships, and work within a community-focused environment that emphasizes evidence-based, meaningful outcomes.
