Massapequa Center Rehabilitation & Nursing is committed to improving our residents’ quality of life and independence through exceptional rehabilitative services and compassionate, patient-centered care. We are seeking a diligent and skilled Occupational Therapist (OT) to join our interdisciplinary team at our skilled nursing facility in Amityville, NY .

This is a full-time position with an 8:00 AM–4:00 PM schedule , with some flexibility. No weekends are required.

Job Responsibilities Include:

• Provide occupational therapy services to residents

• Develop and improve the skills needed for residents to perform daily activities and maintain independence

• Accurately complete and document all treatment procedures and functional mobility tasks in a timely manner

• Establish individualized plans of care based on each resident’s needs, abilities, and goals

• Apply appropriate interventions to maximize residents’ performance in activities of daily living and instrumental activities of daily living

• Monitor resident progress and make adjustments to treatment plans as needed

• Prepare documentation, progress notes, reports, and discharge summaries as required

Qualifications Include:

• Valid New York State Occupational Therapist license and credentials

• SNF/Long-Term Care experience preferred

• Minimum 1 year of occupational therapy experience
