Allied Technician - Physical Therapist

Physical Therapist (PT) - Home Health
We are seeking a dedicated and experienced Physical Therapist (PT) to join our home health team. The ideal candidate will possess a strong clinical background in home health, delivering patient-centric care in a variety of settings.
Key Responsibilities
- Conduct thorough patient assessments and develop individualized treatment plans.
- Provide physical therapy interventions to enhance mobility and functional abilities.
- Utilize OASIS assessments for patient documentation and care planning.
- Collaborate with healthcare professionals to ensure comprehensive patient care.
- Educate patients and their families regarding rehabilitation exercises and home safety.
- Maintain accurate and timely documentation in compliance with regulatory standards.
Qualifications
- Minimum of 6 months to 1 year of recent home health experience required.
- OASIS experience is essential.
- Current and valid Physical Therapist license.
- Compassionate and empathetic approach to patient care.
- Strong communication and interpersonal skills.
Work Schedule
This position offers a day shift with 5 eight-hour workdays, typically from 08:00 AM to 04:30 PM.
Additional Information
- Mileage will be reimbursed at the IRS rate.
