Visiting Rehab and Nursing Services (VRNS) is seeking a compassionate and dependable Physical Therapist (PT) to join our growing home healthcare team in Sandwich, Massachusetts.

Your Role as a Home Care Physical Therapist (PT):

As a Physical Therapist (PT) with VRNS, you’ll provide skilled physical therapy services directly in patients’ homes. You’ll work closely with patients, caregivers, and their care teams to improve mobility, strength, independence, and quality of life. Your responsibilities may include:

• Providing comprehensive physical therapy evaluations
• Developing individualized treatment plans based on each patient’s needs
• Providing therapeutic exercises and functional mobility training
• Assessing patients’ home environments and recommending appropriate safety modifications
• Educating patients and caregivers 
• Monitoring patient progress and modifying treatment plans as needed
• Collaborating with physicians and other members of the care team
• Completing accurate and timely electronic documentation

Need to Have:

• Current Massachusetts Physical Therapist (PT) license
• Minimum of one (1) year of clinical nursing experience
• Afternoon availability

• Previous home care experience
• Monday–Friday availability
• Weekend availability
