Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 32

Requirements:

• Flu vaccine is required
• Must reside outside of 100 miles from the facility

Compliance / Notes:

• No more than 3 days of time off may be requested per 13-week contract
• Facility may cancel up to 3 shifts per 13-week assignment; guaranteed hours will not apply.
