This role focuses on supporting family satisfaction, coordinating non-clinical service needs, and ensuring clear communication between families and internal care teams. The ideal candidate is compassionate, organized, and thrives in a fast-paced, people-focused environment.Key ResponsibilitiesServe as a main contact for families, providing timely and professional communicationSupport family on boarding following service initiationCoordinate non-medical service changes, including schedules, hours, and caregiver preferencesAddress family questions, concerns, and service feedback with empathy and professionalismCollaborate with scheduling and care team staff to ensure continuity of careDocument family interactions and updates accurately in company systemsProactively identify and help resolve potential service issuesSupport family retention and long-term satisfactionWhat This Role Does Not IncludeNo hands-on caregivingNo medical or clinical responsibilitiesNo medication administration, or nursing dutiesQualificationsPrevious experience in social work, case management, family services, healthcare support, or care coordination preferredStrong communication and interpersonal skillsExcellent organization and follow-up abilitiesAbility to handle sensitive situations with professionalism and discretionComfortable working independently and as part of a teamProficient with basic computer systems and documentation toolsCompensation & BenefitsAnnual salary: $40,000–$50,000 (based on experience)Health insurance eligibility after 90 days of employment401(k) eligibility after 12 months of employmentPaid training and ongoing supportOpportunity for growth within a mission-driven organizationWhy Join UsWe are committed to supporting families and aging adults with dignity, respect, and compassionate care.
