Children’s Health Home Care Manager

Pay Rate : $24–$26 per hour, Competitive Based on Experience

Job Type : Full-Time | In-Person / Office-Based with Community Visits

Location : Brooklyn, NY

About the Role

Doral Health & Wellness is seeking a detail-oriented, compassionate, and experienced Children’s Health Home Care Manager to support children and families enrolled in the program.

The Care Manager will be responsible for coordinating comprehensive health and community-based services, developing and monitoring care plans, maintaining accurate documentation, and collaborating with families, healthcare providers, schools, and community service agencies to ensure children receive the appropriate services and support.

Candidates must have prior experience working with Children’s Health Home care management.

Key Responsibilities

• Provide care management services to children enrolled in the program
• Coordinate medical, behavioral health, educational, and community-based services
• Develop, implement, and monitor individualized care plans
• Conduct regular outreach and follow-up with children and their families
• Coordinate with healthcare providers, schools, social service agencies, and other community partners
• Assist families with accessing Medicaid-covered services and community resources
• Monitor client progress and identify barriers to care
• Maintain accurate and timely documentation in EHR and care management systems
• Ensure compliance with Medicaid, Children’s Health Home, and organizational requirements
• Participate in case conferences and multidisciplinary care coordination meetings
• Conduct community visits as needed to support clients and families

Qualifications

• Required: Previous experience in Children’s Health Home (CHH) care management
• High school diploma required; Bachelor’s degree in Social Work, Human Services, Psychology, Public Health, or a related field preferred
• Experience coordinating services for children and families
• Knowledge of Medicaid and community-based healthcare services
• Familiarity with Children’s Health Home policies, care plans, and documentation requirements
• Experience with EHR or care management systems
• Strong organizational, communication, and time-management skills
• Ability to work collaboratively with families, healthcare providers, and community agencies
• Ability to conduct community visits when required
