The Role We’re looking for a compassionate, collaborative Family Nurse Practitioner (FNP) to support our growing clinic and provide high-quality, patient-centered primary care. In this role, you’ll treat the whole person, addressing physical health while working alongside our behavioral health team to support patients with complex, overlapping needs. You’ll play an important part in our integrative model, ensuring care is coordinated, accessible, and genuinely supportive. What You’ll Do Provide comprehensive primary care, including preventive services and chronic disease management Diagnose and treat acute and chronic medical conditions using evidence-based practices Develop and manage individualized treatment plans in collaboration with patients Incorporate a whole-person approach, considering behavioral health, lifestyle, and social factors Provide patient education on conditions, medications, and wellness strategies Monitor patient progress and adjust care plans as needed Maintain accurate, timely, and thorough documentation in the EHR Collaborate closely with psychiatric providers, therapists, and administrative staff to ensure coordinated care Participate in care planning, case consultations, and team collaboration Respond to patient messages, refill requests, and follow-ups in a timely manner Uphold all clinical, ethical, and regulatory standards of practice What We’re Looking For Active, unrestricted FNP license in Colorado Board certification as a Family Nurse Practitioner Active DEA registration Current, valid BLS certification Interest or experience in integrative, trauma-informed care strongly preferred Strong clinical judgment, communication, and interpersonal skills Compassionate, patient-centered approach Ability to work independently while collaborating within a multidisciplinary team Comfortable in a fast-paced, growing clinic environment Experience with Practice Fusion preferred Schedule Monday – Friday, 8:00 AM – 5:00 PM with an hour lunch (12:00 PM - 1:00 PM) Perks & Benefits Health, dental, and vision insurance Group supplemental insurance: hospital indemnity, accident expense, critical illness, and cancer coverage Group life insurance Lyric Virtual Health package (virtual primary & urgent care, mental health support, wellness services) Paid sick leave, vacation, and mental health days (for full-time team members) Paid holidays (for full-time team members) One-on-one mentorship and continuing education opportunities Why You’ll Love Working Here We’re a people-first workplace where inclusivity, support, and growth are integral to our culture, for our patients and each other.
