- Performing detailed patient evaluations, ordering necessary diagnostic procedures, and creating personalized treatment plans
- Diagnosing and managing acute, episodic, or chronic health issues in collaboration with physicians or autonomously, as permissible by your credentials
- Guiding patients and families on wellness, preventive care, and chronic disease management
- Prescribing medications and therapies in alignment with your scope of practice
- Engaging in follow-up care and chronic disease management to ensure optimal patient outcomes
- Working collaboratively within multidisciplinary teams to provide integrated, comprehensive care
- Recording thorough and accurate information within electronic medical records (EMR)
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Graduation from an accredited Nurse Practitioner training program
- Active Nurse Practitioner licensure in Colorado
- National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or a related specialty
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the capability to obtain within 90 days)
- Strong clinical judgment fused with a compassionate approach to patient care
- Exceptional communication abilities engaging with patients, families, and healthcare teams
- Flexibility in dynamic, patient-centered settings
- Familiarity with EMR platforms and telehealth technologies
- A collaborative outlook and dedication to evidence-based practice
