Aventura at Creekside is currently seeking a PRN Speech-Language Pathologist (SLP) to join our dedicated and compassionate in-house rehabilitation team !

• Provide speech-language pathology services to residents and patients experiencing communication, cognitive, and swallowing disorders

• Evaluate patients and develop individualized treatment plans to improve communication, cognition, swallowing, and overall function

• Deliver treatment in accordance with the established plan of care

• Educate and support patients, families, and caregivers on therapy programs and progress

• Monitor and report patient responses to treatment interventions

• Maintain accurate and timely documentation in compliance with company policies and state/federal regulations
