Cancer Screening Patient Navigator-1

Position Summary The cancer screening navigator will assist patients, families and caregivers with understanding and navigating the screening options available to identify cancer risks when they are more easily treatable and support the patient through the screening and follow-up journey. This role performs direct outreach to patients, families and caregivers to provide culturally appropriate education on the importance of cancer screening, assist them with accessing screening services and connect patients to other CHW and other resources as needed. Navigators will also partner with patients to identify and address any barriers or challenges that may prevent screening completions. This role will specifically support the Building Connections Research Grant. As such the role will be trained on 1‑2 interventions to help recruit patients, offer these interventions to enrolled patients, maintain post‑session notes and tracking in REDCap, and participate in weekly supervision with the assigned clinical supervisor for pilot cases when the interventions launch in the following areas: Cancer screening, Obesity and Mental health. Key Responsibilities Serves as central contact for physicians, medical staff and Population Health team to assist patients navigating the cancer screening journey. Facilitates flow of information between patients, providers and services, and cooperates with providers to process referrals. Provides one‑on‑one education on collection processes for specific cancer screenings and offers language‑appropriate materials. Coordinates multiple cancer screening appointments and related services such as radiology or anesthesia. Supports families in follow‑through and referral processes. Facilitates scheduling of additional testing or procedures and coordinates clinician services such as interpreter services. Obtains and updates patient medical records in the chart and EHR. Provides timely support for patients lacking social or financial resources, referring to Patient Financial Services when needed. Attends trainings and professional development to maintain knowledge of current cancer screening guidelines and resources. Documents patient information—demographics, scheduled visit dates, barriers—in EMR and patient tracking database. Enters encounter details and notes into tracking database and/or EMR promptly. Verifies and updates insurance information during scheduling. Proactively contacts patients to resolve and follow‑up potential barriers for screening completion. Provides general clerical support: filing, appointment scheduling, photocopying, faxing, mailing, reminder calls, and maintaining contacts database. Programmatic Functions Identifies system deficiencies and collaborates to fill gaps. Escalates patient issues to appropriate team members. Develops relationships with other clinical‑based navigators and coordinators. Provides and receives constructive feedback. Contributes new ideas to impact the program. Qualifications Required Education and Experience Minimum of a High School diploma or GED. Preferred Education and Experience Experience working with patients in a healthcare or community‑based setting. Previous customer service experience. Completed Motivational Interviewing training. Knowledge, Skills & Abilities Ability to work in a multicultural, diverse environment as part of a health care team. Strong oral and written communication, interpersonal, organizational, and record‑keeping skills. Bilingual or multilingual skills (Albanian, Spanish, Haitian‑Creole, Greek) are a plus. Ability to handle multiple tasks simultaneously. Ability to work independently and as part of a team. Confidentiality and sensitivity to cultural differences. Empathy and ability to coach patients in navigating the healthcare system. Knowledge of Microsoft Excel and Word. Ability to interpret and record data and understand basic medical terminology. Compensation $22.84 – $31.97 per hour. In addition, BMCHS offers benefits and other compensation as described.
