Work Environment

This role is primarily onsite, where the Physical Therapist will deliver personalized care directly in patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients by reviewing their medical history, conducting physical assessments, and identifying functional limitations and rehabilitation goals.
• Develop and implement individualized treatment plans based on assessment findings and evidence-based practices.
• Communicate clearly with patients and their families regarding evaluation findings, treatment protocols, and expected outcomes, ensuring they are fully involved in the goal-setting process.
• Monitor patient progress and modify treatment plans as necessary to ensure optimal patient outcomes.
• Document all evaluations, progress reports, and treatments in compliance with regulatory and organizational standards.
• Educate patients on injury prevention and exercise techniques to promote autonomy in their health management.
• Collaborate with other healthcare professionals, caregivers, and family members to facilitate a holistic approach to patient care.
• Maintain up-to-date knowledge of best practices in physical therapy and participate in ongoing professional development opportunities.

Patient Population / Client Focus

In this role, you will work with a diverse population of patients, including elderly individuals recovering from surgeries, individuals with chronic pain conditions, and those requiring rehabilitation after injuries. Your empathy and ability to adapt treatment methods will be essential in addressing the unique needs of each client, promoting not only physical recovery but also emotional and psychological support.

Qualifications

• A Bachelor's degree in Physical Therapy or a related field is required.
• Current state licensure or eligibility for licensure as a Physical Therapist in Florida.
• Strong interpersonal and communication skills, with the ability to build relationships with patients and their families.
• Compassionate understanding of the physical and emotional challenges faced by patients are essential.

Preferred Experience

• Experience in home care settings or outpatient therapy is highly desirable.
