Nurse Practitioner, Adult and Gerontology

Provide high-quality primary care in a high-touch, person-centered care model Diagnose, treat, and manage acute episodic and chronic illness Conduct wellness visits and detect changes early Provide timely treatment and health education Develop and communicate treatment plans to residents, providers, caregivers, and others Order and interpret laboratory and diagnostic testing and adjust treatment plans Maintain compliant documentation in the EMR software Collaborate with operations, clinical leadership, PCPs, interdisciplinary teams, and specialists Support value-based care initiatives Provide an excellent customer experience Complete required monthly, quarterly, annual, and acute visits Participate in family conferences and provide advanced care planning and education Address pharmacy recommendations and pharmacist communications Coordinate care with telemedicine providers Provide face-to-face coverage at assigned facilities during work hours Follow professional standards, state and federal regulations, guidelines, and laws Requirements Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate National certification from AANP or ANCC Active, unrestricted state license Active DEA license US work authorization 2+ years of clinical experience as a Nurse or Nurse Practitioner in a geriatric, adult, or family practice setting Ability to work in a flexible environment and schedule Passion for providing healthcare to senior and low-income populations Strong oral and written communication skills Detail-oriented and able to work independently and in a team setting Customer service approach with teams and residents Strong time management skills Ability to prioritize and meet competing deadlines while achieving goals Core Competencies Demonstrates expertise in providing high-quality primary care, managing acute and chronic illnesses, and developing treatment plans while ensuring compliance with regulations.
