Patient Intake & Care Coordination Specialist (Hybrid)

OneHome, located in Miramar, FL, seeks an Intake Coordinator to support post-acute care pathways. You will review referrals, verify demographics, assign SOC starters to RNs, and input cases into the system following CMS policies.
