Occupational Therapist — Full-Time & Per Diem

Position Summary

Uvalde Healthcare & Rehab Center is seeking compassionate and motivated Occupational Therapists (OTs) to join our rehabilitation team in an in-house subacute rehabilitation setting .

The Occupational Therapist will provide skilled occupational therapy services to residents and patients recovering from illness, injury, surgery, hospitalization, or functional decline. The OT will work collaboratively with nursing, physical therapy, speech therapy, physicians, and other members of the interdisciplinary team to help patients maximize independence, safety, and quality of life.

Both full-time and per diem opportunities are available.

Key Responsibilities

• Evaluate patients to determine functional abilities, limitations, and rehabilitation needs.
• Develop individualized occupational therapy treatment plans based on physician orders, evaluations, patient goals, and applicable regulations.
• Provide skilled occupational therapy interventions to improve strength, coordination, balance, mobility, cognition, activities of daily living, and functional independence.
• Address activities of daily living (ADLs) and instrumental activities of daily living (IADLs), including dressing, bathing, grooming, feeding, toileting, transfers, and other functional tasks.
• Provide appropriate therapeutic activities and exercises to support recovery and maximize functional outcomes.
• Recommend and train patients in the use of adaptive equipment, durable medical equipment, and compensatory strategies as appropriate.
• Educate patients, residents, families, and caregivers regarding treatment plans, safety techniques, home programs, and strategies for continued progress.
• Monitor patient response to treatment and modify interventions as clinically appropriate.
• Complete evaluations, daily treatment documentation, progress notes, reassessments, discharge summaries, and other required documentation accurately and timely.
• Participate in interdisciplinary care planning and communicate patient progress and recommendations to the appropriate members of the care team.
• Assist with discharge planning and provide recommendations for continued therapy, equipment, and support as appropriate.
• Maintain compliance with facility policies, professional standards, payer requirements, and applicable federal and Texas regulations.
• Maintain professional relationships with patients, families, caregivers, and facility staff.
• Promote a safe, respectful, and patient-centered rehabilitation environment.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Texas Occupational Therapist license in good standing.
• Current CPR/BLS certification, or ability to obtain as required by the facility.
• Experience in skilled nursing, subacute rehabilitation, inpatient rehabilitation, hospital, or geriatric settings is preferred but not required.
• Strong clinical, communication, organizational, and documentation skills.
• Ability to work effectively as part of an interdisciplinary rehabilitation team.
• Compassionate, dependable, and committed to providing high-quality patient care.

Full-Time Position

The full-time Occupational Therapist will provide consistent skilled therapy services as part of the facility's ongoing rehabilitation program and will participate in interdisciplinary collaboration, care planning, documentation, and discharge planning.

Per Diem Position

The per diem Occupational Therapist will provide coverage based on facility census, patient needs, scheduling requirements, and therapist availability.

Per diem opportunities may be well suited for therapists seeking a flexible schedule or additional clinical hours.
