Assessment Counselor

Position Overview Provides therapeutic interventions aimed at fostering personal growth and resilience in clients with behavioral health diagnoses. Conducts ongoing assessments of client needs, maintaining accurate documentation, and developing personalized growth plans tailored to therapeutic requirements. This role involves significant decision‑making regarding the implementation of interventions and adjustments to treatment plans based on client progress. Position: Full time. Pay Range: $48,300.00 – $67,700.00. Key Responsibilities Engage with newly assigned clients and their families by documenting contact within 2 business days of referral. Conduct intake for clients entering services, ensuring understanding of all forms and obtaining necessary signatures. Develop individualized care plans that are person‑centered, family‑centered, and service‑centered. Assist clients in linking with internal and external resources as outlined in service plans. Maintain documentation of services provided and submit for review as required. Monitor client progress in treatment and participate in service plan reviews. Provide community support, care coordination/case management, and crisis intervention services. Knowledge, Skills & Abilities Knowledge of community geography and resources, including agency resources. Skill in interviewing, assessment, and counseling. Proficiency in customer service techniques with effective resolution of client issues. Advanced problem‑solving skills; demonstrated successful project outcomes. Familiarity with digital communication systems, including Electronic Health Record for managing client interactions. Ability to conduct thorough assessments and form diagnoses using standardized tools. Expertise in virtual communication platforms ensuring clear and effective remote interactions. Strong analytical skills applied in evaluating individual needs and resources. Qualifications Education: Master’s Degree in Human Services field, licensure eligibility required. Experience: 1 year of experience. Licensure: licensure eligibility required. Physical Requirements Standing – 20% of time. Sitting – 80% of time. Squatting – occasional. Driving – occasional.
