Responsibilities

• Deliver high-quality physical therapy assessments and treatment plans.

• Develop and execute individualized therapy programs aligned with patient goals.

• Ensure compliance with safety, ethical, and regulatory standards.

• Document progress notes and maintain accurate patient records per policy.

• Track patient outcomes and adjust plans to optimize recovery timelines.

• Collaborate with multidisciplinary team to coordinate care and referrals.

• Engage in continuous learning to stay current with therapy best practices.

• Demonstrate measurable results through weekly reports and KPI tracking.

• Participate in pipeline planning to identify and pursue growth opportunities.

• Provide patient and family education to support at-home care.

• Mentor junior staff or interns to foster team development.

Experience/Qualifications

• Licensed Physical Therapist with valid state license.

• Provide comprehensive PT assessments and individualized treatment plans.

• Demonstrate measurable outcomes with weekly KPI reporting.

• Document progress notes and maintain accurate patient records per policy.

• Ensure compliance with safety, ethical, and regulatory standards.

• Collaborate with multidisciplinary team and coordinate referrals.

• Engage in continuous learning; stay current with best practices.
