RN Hospice Case Manager

Position Summary LifeSource Hospice is seeking a compassionate and experienced RN Hospice Case Manager to join our interdisciplinary team serving patients in Bergen County, NJ. This role is responsible for managing a caseload of hospice patients, coordinating care across disciplines, and ensuring compliance with hospice regulations and standards. The ideal candidate has hospice, home health, or long-term care experience and is comfortable working independently in the field. Key Responsibilities Manage a hospice caseload of approximately 15 patients Conduct comprehensive nursing assessments and develop individualized plans of care Provide skilled nursing visits and symptom management Coordinate care with physicians, social workers, chaplains, aides, and other IDT members Ensure documentation meets CHAP standards and regulatory requirements Utilize WellSky EMR for timely and accurate charting Perform medication reconciliation and coordinate e-prescribing via BetterRx Provide patient and family education related to disease progression and end-of-life care Participate in IDT meetings and care planning Collaborate with facilities and community partners Travel to patient homes and facilities within Bergen County Qualifications Active NJ RN license in good standing Minimum 1 year experience in hospice, home health, or long-term care Familiarity with CHAP hospice standards preferred Experience with WellSky EMR and BetterRx a plus Strong clinical assessment and communication skills Compassionate, patient-centered approach Reliable transportation and valid driver’s license Ability to work independently and manage time effectively Schedule Full-time Monday–Friday On-call rotation (one weekend per month in Bergen county vicinity) What We Offer Competitive salary Medical/Dental/Vision Benefits 401k with matching Mileage reimbursement Supportive interdisciplinary team Meaningful, patient-centered work Opportunities for professional growth #J-18808-Ljbffr Lifesource-Hospice
