SERENITY HOSPICE CARE
Serenity Hospice Care in New Jersey seeks a compassionate RN Case Manager to deliver patient-centered end-of-life care and coordinate plans with families and the care team. You will conduct initial assessments, monitor nursing needs, develop care plans, and support the patient and family through the course of their illness, traveling to Bergen County as needed to ensure continuity of care.
