Key Responsibilities\n

• Conduct evaluations and psychosocial assessments to identify the needs of clients and their families.

• Participate in the development and execution of Individualized Family Service Plans (IFSPs), ensuring that each plan is tailored to the unique circumstances of the family.

• Actively monitor outcomes and progress as a team member, contributing to interventions that promote client well-being.
\n
• Offer emotional support and counseling to families, assisting them with coping strategies related to the terminal illness or the loss of a loved one.
\n
• Provide comprehensive information and resources regarding community services and support systems applicable to family needs.
\n
• Implement evidence-based strategies following the Natural Learning Environment Practices to facilitate optimal child development.
\n
• Establish strong and trusting relationships with clients, families, and team members to foster a supportive environment.
\n
• Ensure compliance with all Federal and State regulations related to social work and hospice care practices.

Patient Population / Client Focus\n

The primary focus of this position is to support hospice clients and their families, encompassing those experiencing end-of-life situations. Our clientele includes a diverse mix of backgrounds and needs, requiring a culturally sensitive approach to care. Our clinical social workers advocate for clients’ choices and support them in navigating complex emotions associated with prenatal, pediatric, and end-of-life experiences.

Qualifications\n

• A Master's Degree in Social Work (MSW) or Doctorate in Social Work (DSW) from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) designation in the state of Pennsylvania, issued by the Department of Public Health.

• Current and valid driver's license, with the ability to travel as needed for home visits.
\n
• Exceptional organizational skills, coupled with effective time management capabilities.

• A strong commitment to adapting treatment plans to fit individual client needs, demonstrating flexibility in approach.

Preferred Experience\n
