▪ Radius rule: Traveler’s permanent address must be 50 miles outside of home unit

● Department Information:

o 6 visits a day

o There is not a max mileage to drive. The average is approximately 60-80 miles a day. Mileage is reimbursed weekly.

o Minimum Years of Experience required? 1 year Home Health

o Will you accept a first-time traveler? Yes, if seasoned to home health

● Patient Types/Common Diagnoses:

o All Adults

o Chronic diseases, Orthopedics/Ortho Surgery including total hips/knees, falls, weaknesses, CHF, COPD, occasionally stroke patients

o Teammates will travel throughout the below areas:

▪ Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy and cities in between

● Required License, National Certification, Certs:

o Driver's License and proof of Insurance required

o PT: BLS; State Licensure required

● Skills required:

o PT: Gait training, balance training, non-pharmaceutical pain management, create plan of care to get patients to highest level of functioning in the most efficient way, good communication with clinical team, recognizing signs and symptoms to prevent readmissions

● Support within the Department:

o Departmental leadership

o Schedular

o Clinical Manager

o Clinical Supervisor

o Peers/Clinical Team

● Technology/Equipment:

o EPIC Rover – view only

o   Focura - will upload discharge instructions and other important docs such as wound pictures

o Samsung note 9 phones

o Home Care Home Base - required

● Orientation (facility and unit) :

o NEO

o 5 days orientation

● Scheduling:

o Weekend rotation none

o No call

o No major holidays

o RTO: Manager approval

o Schedule cycle: every 13 weeks

● Additional Information:

1. Do they schedule their own appointments with this position? - They will schedule the visit with the patients the day prior to the visit planned.
2. Do they still get paid for cancelled appointments? Cancelled appts are not counted as visit equivalents. They are required 30 VE a week
3. Will a company car be available? No
4. Is a company cell phone provided? A phone is provided and is used for documentation and calling patients/providers.
