The MDS (Minimum Data Set) Coordinator has a heart for older persons and is passionate about enhancing the quality of life and care for those at . As an integral member of the nursing team, the MDS Coordinator ensures the accurate picture of the resident managing all aspects of the MDS assessment and care planning process in accordance with current federal, state, and local standards alongside the mission, vision, and values of the Little Sisters of the Poor. This position intentionally advances person-centered care and services irrespective of the presence of chronic and/or acute disease. These standards and guidelines are designed to attain the highest possible degree of quality of life and quality of care across multi-levels of aging services; ranging from residential to skilled nursing. Holy Family Residence is licensed for 73 skilled nursing beds to complete the senior living community. Requirements and Qualifications Active license with the state as a Registered Nurse in good standing. Proficiency in computer usage and software applications, including, but not limited to, electronic medical records and Microsoft Office. Ability to follow and give written and oral directions. Proficient in English. Ability to work both independently and collaboratively. Communicate effectively by dealing tactfully and respectfully with all persons. Flexibility and a willingness to adapt to change in an ever-evolving environment. Ability to demonstrate continual learning skills, effect changes in approach to care based on established, evidence-based practice. Possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, polices and procedures, etc., that are necessary for ensuring the accurate and timely completion of the RAI documents. Essential Functions Manuals, Policies/Procedures and Mission Integration materials for the Little Sisters are located in the Director of Nursing Services Office. A personal copy of current Employee Handbook, the Home’s Supplement and Benefit Guide are provided at time of hire. Gather the MDS (Minimum Data Set) via the Resident Assessment Instrument (RAI) process, which assesses a resident’s behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized care plan, including measurable objectives and time tables, that considers the resident holistically. The MDS Coordinator then leads the interdisciplinary team in developing a comprehensive resident assessment and care plan for each resident, ensuring care plan guidelines for each resident are followed and compliant with ethical standards and Medicaid and Medicare requirements. Serve as the expert resource for thePatient-Driven Payment Model (PDPM) and responsible for complying with ethical standards when setting Assessment Reference Dates (ARDs), completing assessments, and upholding Medicaid and Medicare requirements. Coordinate, conduct, and complete the MDS, individualized resident care plans and conferences in accordance with the RAI Manual, State, and Federal requirements. Assure effective communication and education with the interdisciplinary team as it relates to documentation guidelines and regulatory requirements, including attending physicians and nursing staff. Work closely with hospital discharge planners and physicians to obtain accurate and complete documentation to support ICD-10-CM diagnosis coding. Support timely, accurate, and complete documentation of clinical information. Complete/submit PASRR documents and ensure appropriate follow-up. Be highly involved in determining skilled level of care for Medicare residents and procuring required Medicare-specific documentation; be responsible for physician certification/recertification of a skilled level of care throughout the Medicare stay; be involved in making Medicare eligibility determinations. Coordinate care with case managers for residents utilizing managed care, health maintenance organizations (HMOs), commercial insurance, and other alternate payment models. Actively participate in case mix audits and in Facility surveys (inspections) made by authorized government agencies. Access and provide Quality Measures, CASPER, and Case Mix reports as requested. Participate in the monthly Triple Check meeting; prepared with necessary reports in order to complete a final check or audit to ensure all requirements of Medicare billing are in place prior to submission of the claims. RESIDENT CARE AND QUALITY OF LIFE Ensure that nursing services assure flexibility to staff to meet residents’ preferences. Maximize resident quality of life and quality of care from the time of admission to discharge/end of life. This is achieved by maintaining a daily proximity to the residents, in particular those with acute nursing needs. Participate in nursing rounds to promote prompt assessment of resident behavior and/or response to interventions. Collaborate closely with the Little Sister hospitalist to meet each Resident’s preferences, needs and desires as evidenced in a personalized “Plan of Care”. Play a pivotal role in maintaining a dynamic interdisciplinary approach, since all departments impact the security, safety, and happiness of the residents (i.e. social and business services, food service, spiritual/therapeutic recreation/activity programs, laundry, physical environment). Identify a just balance of individual Residents’ rights and choices, personal comfort and safety within a congregate setting. Participate in fostering a dementia-friendly culture for residents with memory impairment. ADMINISTRATIVE / REGULATORY Assure that substantial compliance with applicable federal, state, and local standards and regulations for nursing services aligns with the Little Sisters of the Poor mission of respect for life and compassionate care to the elderly. Adhere to all administrative and nursing policies and procedures, including but not limited to HIPAA, safety/OSHA, infection prevention, residents’ rights, quality improvement and assurance indicators and those contained in the employee handbook. Exhibit respect for each individual by assuring confidentiality of personal information and events. Demonstrate a willingness to pitch in, especially during times of resident and/or staff emergency, public health crises, staffing shortages (serve as a staff nurse when staffing levels necessitate in order to meet the needs of the residents). Serve as a member of the Quality Assurance Committee, meeting at least quarterly, as a key contributor, providing insight and analysis of quality metrics for the QAPI program. Support a strong and united nursing administrative team by identifying and fostering complementary strengths, talents, and clinical skills.
